Website Change Request Form
Please read the instructions below before completing this form.
Instructions
- The MTM Website Change Request Form MUST be submitted by the 3rd of every month for changes to be updated for the upcoming month.
- After the Website Change Request has been submitted, an email will be sent IMMEDIATELY to the contact email submitted on the form below to confirm the request was successfully submitted. If an email is not received, the Website Change Request was not successfully submitted.
- A Receipt Notification will be sent via email within 24 hours.
- A Completion Date Notification will be sent via email within 72 hours.
- All pictures must be submitted in .jpeg or .png format only and sent as an attachment to email.
- All videos must be submitted in .avi format only and sent as an attachment to email.
- Please proof prior to submission. Proofing/Editing will not be done by MTM.
